The short version: a good dog heart disease diet has two jobs, not one — restrict sodium across the whole day (treats and chews included) and protect muscle with plenty of high-quality protein. Cutting protein to “feed light” is the classic mistake that quietly makes a cardiac dog worse.
Heart disease is one of the most common chronic diagnoses in ageing dogs. In smaller breeds it usually means degenerative mitral valve disease (also called myxomatous mitral valve disease, or MMVD); in larger breeds it is more often dilated cardiomyopathy (DCM).
Different diseases, same downstream problem: a heart that pumps less efficiently. That is where food comes in — not as a cure, but as day-to-day support for the two challenges that follow. One is about sodium and fluid. The other is about muscle.
Building a dog heart disease diet around two goals
Almost everything worth knowing about a dog heart disease diet fits under two headings: lower the fluid-retaining load (sodium) and defend lean body mass (protein and calories). Get one right and ignore the other, and you have solved only half the problem.
Sodium: why it is the cornerstone
As cardiac output falls, the body misreads the drop as low blood volume and switches on the renin-angiotensin-aldosterone system (RAAS). Aldosterone tells the kidneys to hold on to sodium, and water follows the sodium.
Blood volume climbs, and a heart that is already struggling now has to move even more of it. The extra fluid backs up — into the lungs as pulmonary oedema, or into the abdomen as ascites — which is exactly the congestion in congestive heart failure.
Restricting dietary sodium eases that load, which is why it has long been the cornerstone of feeding cardiac patients. Dogalicious Heart Care holds sodium at 0.14% on a dry-matter basis (chicken) or 0.22% DM (fish), with no added salt.
One nuance worth knowing: sodium restriction is matched to the stage of disease. Early on, only mild-to-moderate restriction is needed; aggressive restriction is generally reserved for advanced failure. That is one more reason to let your vet set the target rather than cutting sodium to zero on your own.
“I never add salt” does not solve it
Most of the sodium a dog with heart disease consumes never comes from the salt shaker. It is already inside the food. Commonly overlooked sources include:
- Cheese cubes and cheese sticks
- Jerky and meat-strip treats
- Ham, luncheon meat and sausage
- Human canned foods and instant broths
- Some dental chews and flavoured chews
- Certain supplements and medication excipients
Sodium control is about the daily total, not the shaker in your kitchen. A couple of salty treats can quietly cancel out an entire bowl of low-sodium food.
A worked example: budgeting sodium for a 10 kg dog
Picture a 10 kg dog eating a low-sodium therapeutic meal. The meal is doing its job — sodium is kept deliberately low. Then, over the day, the dog also gets two small strips of jerky, a corner of cheese and a couple of flavoured dental chews as “just treats”.
Individually those feel trivial. Together, processed treats like these can carry more sodium than the whole day’s meals combined — undoing the diet you are paying for. A useful rule of thumb from veterinary nutritionists: treats should make up no more than about 10% of daily calories, and for a cardiac dog they should also be low in sodium.
Swap the jerky for a few pieces of the dog’s own food or a slice of plain cooked chicken, and the sodium budget survives the day intact.
Muscle: the counter-intuitive part
This is the least intuitive point in cardiac nutrition. A 2019 study in the Journal of Veterinary Internal Medicine followed 269 dogs with congestive heart failure:
- 130 (48.3%) had cardiac cachexia — that is, muscle loss
- Dogs with cachexia had a median survival of 233 days
- Dogs without had a median survival of 321 days (P=.04)
- On multivariable analysis, cachexia was an independent risk factor for shorter survival
Nearly half of dogs in heart failure are losing muscle, and that loss is itself associated with a worse outlook. Importantly, cardiac cachexia is driven by inflammation and altered metabolism — not simply by eating too little — so you cannot always reverse it by feeding more. But you can absolutely make it worse by feeding less protein.
So what a cardiac dog needs is not less food or blander food. Protein must stay generous — we hold it at 42–46% DM. What comes down is sodium, not meat.
Something you can do tonight: run your hand from the shoulder blade along the spine to the loin and feel the depth of muscle. Repeat every two weeks. Being able to feel ribs and vertebrae easily is not necessarily healthy leanness — it can be early muscle wasting.
Myth versus fact
- Myth: “Heart disease means less protein.” Fact: unless there is advanced concurrent kidney disease, protein should stay normal-to-high; cutting it accelerates muscle loss.
- Myth: “Low sodium means bland and unappetising.” Fact: appetite matters enormously in cardiac dogs. A palatable low-sodium food the dog actually eats beats a “perfect” diet it refuses.
- Myth: “If I don’t add table salt, the diet is low sodium.” Fact: most sodium is already inside processed foods and treats.
- Myth: “Supplements can replace a cardiac diet.” Fact: taurine or fish oil may help, but they do not lower sodium or supply the protein and calories that protect muscle.
Taurine and omega-3 fatty acids
Two further nutrients target the heart muscle directly.
Taurine is an amino acid tied to heart-muscle function. Some breeds develop taurine-responsive DCM, and diet-associated DCM has been the subject of an ongoing FDA investigation — so we add taurine to the formula rather than assume the meat supplies enough.
Omega-3 EPA and DHA provide cardioprotective, anti-inflammatory support. A 1998 study in the same journal found that dogs with heart failure had altered fatty-acid profiles and that fish-oil supplementation reduced inflammatory mediators — part of the rationale for building EPA and DHA into a cardiac recipe.
The Hong Kong angle
Two things make sodium control harder for Hong Kong dog owners. First, the food we share. Char siu, dim sum, soy-sauce braised dishes, ham and preserved snacks are all high in sodium, and a dog begging at the table in a small flat is hard to refuse.
Second, the humidity. In summer, dogs drink and pant more, and owners are tempted to offer “tasty” hydration in the form of salty broths or human snacks. For a cardiac dog, plain fresh water is the right answer — and any broth should be unsalted and vet-approved.
A practical local habit: set aside a small container of the dog’s own food as the “treat jar”, so family members and domestic helpers always have something safe to hand out instead of a piece of char siu.
A simple weekly routine
- Weigh the food. Eyeballing portions drifts over time; a kitchen scale keeps calories and sodium consistent.
- Log every treat and chew for one week — most owners are surprised by the running total.
- Do the two-week muscle check (hand along the spine) and note what you feel.
- Weigh the dog weekly, same scale and time. A sudden gain can mean fluid retention — report it.
- Watch the resting breathing rate while the dog sleeps. A rate creeping up over several nights is an early congestion sign worth a vet call.
- Keep medication and feeding times consistent, and never change the diet or skip diuretics without asking your vet.

Is low sodium enough for a dog heart disease diet? No. Low sodium reduces the heart’s workload, but protecting muscle mass matters just as much — a low-sodium, low-protein diet can do more harm than good.
Should I cut protein for my dog’s heart? Generally no. Unless your vet is managing advanced concurrent kidney disease, protein should stay normal-to-high; too little accelerates the muscle loss that already threatens cardiac dogs.
